Most teams don't suffer from a lack of effort. They suffer from too much to think about at the same time.
Engineers juggle features, bugs, incidents, dependencies, meetings, tooling and shifting priorities. Product partners balance stakeholder demands, delivery pressure and uncertainty. These aren't individual problems. They're signals that the system is too heavy to navigate easily. You should pay attention to these signals. They indicate where your ways of working need to be simplified or clarified.
